Referral

It is important that we make this referral process as straightforward and accurate as possible for referring agencies. It is imperative that each young person is appropriately placed in the correct service in order to receive the right level of support to match their individual needs.

We will always priorities the needs of the young person being referred and those already placed at our setting. This is to ensure that the requirements and safeguarding needs of each young person are well considered and planned for. As such we will endeavour, to appropriately match each young person to the most suitable placement. 
Each local authority is required to provide a comprehensive referral and risk assessment so as to inform the placement decision making process. This will inform of the circumstances of the young person, their needs, identify any safeguarding concerns and their history. 

Consideration is of course given to the young person’s cultural, religious, communication, and dietary requirements. Following the review of the information and risk assessment (Signs of Safety) we will be able to determine if a placement can be offered.
We are able to consider emergency placements at the setting in such cases as out of hours. For such cases a telephone referral is adequate however a written referral and risk assessment will be required ASAP and no later than 12 hours following placement at the setting.
Our process can be understood by the four straightforward stages presented as below:
Stage One:

Stage Two:
An allocated key worker will be identified in preparation for the young person’s arrival. 
Whenever possible the Key Worker will meet the young person upon arrival to establish a consistent and positive working relationship.
Stage Three:
Within 7 days of residence a meeting will be organised with the management of JenKin support services, the Young Person and Social Worker to discuss the young person’s specific needs and how best to meet them.
Stage Four:
A Key-working meeting will take place within 48 hours from placement (except in exceptional circumstances), this will inform the young person’s Pathway Plan.
